If you are are looking for your annual flu shot we would be happy to administer it for you. If you wish to get your flu shot with us please click the button below to fill out our intake form as best as you can. After you submit your form, please schedule an appointment. Make sure to bring your insurance or Medicare card if you have one.
If you also need a COVID Shot, please follow the link below to fill out the COVID Shot consent form.
ATTENTION: Any child under 3 years must have a prescription for each dose of a vaccine. The parent or guardian of the child must contact their medical provider who must send a prescription into the pharmacy prior to each dose.
PLEASE DO NOT MAKE ANOTHER APPOINTMENT, WE CAN DO BOTH COVID AND FLU SHOTS IN THE SAME APPOINTMENT!
